ELSO Case Report Forms (2.00)

These templates only works under Microsoft Word 97 (version 8.0) , Word 2000 (version 9.0) and not under Word 95 from Office 95.  Requires Windows 95, 98, 2000 or Windows NT 4.0.

Download Version 2.00 of the ECLS Registry Form and Version 1.00 of the ECLS Cardiac Addendum (Word 97/2000 format)

 

This download installs both templates automatically.

To download the forms, click on the above link, and choose to save ELSOsetup2.0.exe to disk. This file is a self-installing program. If you select "Open" during the download, it will download and execute. Execution will place the Word template in the standard template directory. If you are using Windows NT or use a non-standard template directory you will need to designate where the template should be saved.  To determine where you need to save the template, open Word and in the menu item Tools.. Options... under the File Locations tab the User Template directory is displayed.  

 

It will also set up an ELSO program group as well as permit an uninstall through Add/Remove programs in the Control Panel.  Please read all screens as the installation program is in progress.

For advanced computer users who wish to install Version 2.00 of the ECLS Registry Form themselves click here.  Version 1.0 of the cardiac addendum can be downloaded by clicking here.  The files are Word 97/2000 templates (make sure to save as a .dot file extension), and should be saved to the default template folder (designated in the menu item Tools.. Options... under the File Locations tab).
