The H1N1 registry form has been developed to track H1N1 cases that have been placed on ECLS. This form is intended to quickly gather limited H1N1 data to supplement the ECLS registry.

As soon as the H1N1 patient goes on ECMO submit the H1N1 registry form's initial data section. The final data section can be filled out as data become available.

If your center is not a member of ELSO be sure to submit the H1N1CenterRegistration.doc to establish a center identifier which will allow you to submit data to ELSO.

To start submitting data click on the below links for the registry form and instructions set. Save the ELSOH1N1RegistryForm.dot as "ELSOH1N1RegistryForm.dot" and not end with a .DOC. Once the word template has been saved to your computer double click to begin. Complete instructions on filling out the form are in the instruction document.

ELSO receives H1N1 data when a patient goes on ECMO which causes some results to be pending until the forms are updated when the patient goes off ECMO.
